There are many contact forms available for WordPress. I love contact forms that break the mold and let you add custom fields. Not everyone wants only a name, email, and message. Sometimes you want additional information. With these forms you can add elements like drop down lists, radio buttons, check boxes and more.

1. Wufoo

Wufoo forms are very customizable. Even the most novice internet use can use Wufoo with no coding knowledge. You are able to embed the forms in your site.

2. My Contact Form

My Contact From allows you the ability to make your very own contact form with tons of options that most forms have never even thought about including. You can also embed this form code into your blog page, post, or sidebar.

3. CFORMS

Now this is my all time favorite contact form creator. CFORMS is a plugin that I use on this site and my personal blog Riding with Jessica. It complete with field customization using ajax and many stylish templates that are also customizable.
